At Evident Scientific, we prioritize your well-being. That's why we offer all full-time benefits eligible employees Short and Long-Term Disability coverage through New York Life at no cost to you, ensuring your protection during these unforeseen circumstances. This coverage is 100% paid by Evident Scientific, and no action is required to enroll.

Disability coverage provides an income replacement benefit in the event you are unable to work due to a non-job-related illness or accident. The ability to work is typically an employee’s most important financial asset. The Social Security Administration estimates that just over 1 in 4 of today’s 20-year-olds will become disabled before reaching age 67. Once an individual has become disabled for over 90 days, the average length of disability is 4 years.

 

Eligible employees receive employer-paid short-term and long-term disability coverage.

Disability Coverage


Basic Long Term Disability: The federal government considers these benefits part of an employee’s total compensation and designates them as taxable income. On your paycheck you will see an amount (code LTI) under deductions and the same amount under earnings.

File a Disability Claim - Employees


1. Before You File Your Claim

  • Notify your employer if you need to be out of work because of an accident, injury, illness, or to care for a family member, child bonding, or pregnancy.
  • Have the following available:
    • Your Social Security number, date of birth, home address, phone number, and email address
    • Dates of health care provider or hospital/cinic visits and their contact information
    • Workers compensation claim, if applicable

2. File Your Claim

3. Claim/Leave Status

  • Depending on your reason to be away from work, we will provide you addtional FML, state, and/or company/Leave information.

Family Medical Leave Act (FMLA)


If you have been with the company for 12 months, you may be eligible for up to 12 work weeks of unpaid leave per year under the Family and Medical Leave Act (FMLA). FMLA can be used for an illness of your own, care needed for a family member, care for a newborn and certain other medical needs.
